Self-Referral Form for MRI Scan
Please complete this form to request an MRI scan. Your responses will help us assess your eligibility and schedule your appointment. All information provided will be kept confidential.
An MRI (Magnetic Resonance Imaging) is a type of scan that uses strong magnetic fields and radio waves to produce incredibly detailed images of various parts of your body.
An MRI scanner is a large tube containing powerful magnets, the area being scanned will always be in the centre of the scanner.
An MRI scan can be used to examine almost any part of the body and results of your scan can be used to help diagnose conditions, plan treatments and assess how effective a previous treatment has been.
The scanner makes very loud tapping or buzzing noises. This is normal and part of how the MRI scanner works. You will be given a call bell, for any reason you need, you squeeze the call bell, we will stop the scan and attend to you immediately. It’s very important to keep as still as possible during your MRI scan.
MRI scans can take between 15 to 90 minutes.
For some MRI scans, an injection of a clear contrast dye maybe required. Contrast dye is used to highlights some structures more clearly. It will depend on the area being scanned and what we need to see. You will be told beforehand if an injection will be required and you can discuss any concerns you have before the scan starts.
The images will be read and a report will be issued and send back to the referrer.
Any reports will go back via email or post directly to your GP.
To book an MRI scan, we will first need a referral from a GP.
Once we have your referral, we will contact you to make an appointment.

Unfortunately, we do not offer:
- Contrast Angiograms
- Small bowel studies
- Paediatrics
- Temporal Mandibular Joints
Please also be aware that we do not offer an immediate reporting service for querying fractures; please refer to your local minor injuries or A&E department.
